The Upper Scioto Valley kindergarten registration for the 2014-2015 school year will be held on Monday, May 19 and Tuesday, May 20 at the Upper Scioto Valley Local School, McGuffey, Ohio. Times will be between 8:15 a.m. and 2:15 p.m. each day. Please call 419-757-3231 extension 4, before May 12, 2014, to register your child for the kindergarten screening.
At the time of registration you must bring your child and your child’s birth certificate, and immunization records. You must also bring custody papers if applicable. The school will not allow registration without these documents. Registration will take approximately one hour.
Immunizations required for Kindergarten entrance are:
-5 DPT. If the last DPT was before age 4, a 5th dose is required
-4 Polio. If the last Polio was before age 4, a 5th dose is required.
-2MMR-one between 12 and 15 months of age, and a 2nd dose before kindergarten entrance
-3HBV (Hepatitis B)
-2 Varicella (Chickenpox)
If your child has NOT had all of the required immunizations, please call your doctor or go to the Kenton-Hardin Health Department to complete these requirements PRIOR TO kindergarten registration.

It is important to call and pre-register your child, before May 12, 2014, for this kindergarten screening if you are planning to attend the Upper Scioto Valley Kindergarten Program during the 2014-2015 school year.
Note: Due to kindergarten registration, the current kindergarten students WILL NOT have school on Monday, May 19 and Tuesday, May 20, 2014. All other students’ grades 1st through 12th should attend classes on these days as normal.
